Screening Instructions:

Along with your name and contact details, you are required to provide 2 pieces of verification:

• Recent picture of yourself (required)

• Photo ID (option 1)

• Employment verification (option 2)

• Recent provider reference (option 3)

*If sending provider references, please notify them ahead of time for permission to be contacted.

Discretion, privacy, and safety are always a top priority. All screening information is secured upon submission and deleted after our first meeting.
